The possessed blood vial is the item that unlocks the Nyrex the infernal boss pet. It is untradeable. This item is exclusive to Revival RSC and was not available in the original RuneScape Classic.
Item sources
Rolled every time Nyrex the infernal dies, for every player who dealt enough damage to qualify for the kill. The rate gets better the more of that boss you have killed. It will not drop while you already own one, or once you have unlocked the pet, and the drop is announced to the whole server.

Uses
Right-click Unlock and confirm to permanently unlock the pet – a miniature Nyrex the infernal that follows you around. Pets are cosmetic. Right-clicking your pet shows your kill count for that boss and the kill count it dropped at.
::petlists your unlocked pets,::pet <name>summons one and::pet hidedismisses it.- The item cannot be dropped, only destroyed, which puts you back in the running for another.
- If you die with an unhatched item in your inventory it is destroyed.
